Chaffee County dental needs survey online

Please take the Chaffee County Dental needs survey!  The Chaffee County Dental Coalition has been awarded a grant from The Colorado Health Foundation to determine dental needs in Chaffee County and then develop a plan to address these needs.  However, the coalition will never know the needs in the county without citizen input.  Please complete this short survey and e-mail this link to any and all of your adult Chaffee County friends!
